Our Approach
Complex Care Planned Around the Person
Complex care starts with a thorough assessment of the person's health, needs, goals and preferences. We then build a detailed care plan with the person, their family and the healthcare professionals involved. Carers receive training specific to the person they support, and plans are reviewed regularly so care keeps pace with changing needs.
1. Assess
We take time to understand health, routines, risks and personal goals.
2. Plan
We agree a detailed care plan with the person, family and professionals.
3. Train and review
Carers are trained for the person, and the plan is reviewed regularly.
