Your Journey to Arranging Home Care
- 1
1. Get in touch
Call 01384 598958, request a callback or send an enquiry. Tell us a little about the situation and the support you are looking for. There is no obligation. - 2
2. An initial conversation
A member of our team talks with you about needs, routines, preferences and the services that may help. We answer your questions honestly and explain how costs work. - 3
3. Your care assessment
We arrange an assessment, usually at home, with the person who needs care and anyone they would like involved. We look at health, daily routines, safety and personal goals. - 4
4. Your personalised care plan
We write a care plan that sets out what support will be provided, when and how. We agree it with you before anything starts, and you are welcome to ask for changes. - 5
5. Your care begins and is reviewed
We introduce your carers and start support. We check in early on and review your plan regularly and whenever needs change.
